Method of Payment
Total cost for SEGMENT 1 ONLY is $499 or $549 for S1 & S2. Payments can be made online with a credit card. Checks will not be accepted. To register, complete an online registration form. Class size may be limited based on enrollment and registered students will be given first preference. There must be enough for a class. Refund Policy
If a student cancels at least 7 days before the 1st day of class, 100% of the cost paid will be refunded. Within 1 week of class, no refund will be given. In case of misconduct, no refund will be given and student may be expelled.

The parent or guardian certifies that the student is at least 14 years and 8 months by the first day of class, and has no physical handicaps that will interfere with driving an automobile. This must be verified by birth certificate on the first day.

Segment 1 Course Work
Each student will receive 24 hours of classroom instruction, 6 hours of behind-the-wheel (BTW) instruction and 4 hours of observation time in a dual controlled fully insured automobile. Classroom instruction must be a minimum of 3 weeks in length. Requirements to pass the course: 24 hours of classroom instruction, 6 hours of behind the wheel training, 4 hours of observation time, passing of the written state test. The required score to pass the written STATE TEST is 70% or -24 of the 80 question test. BTW instruction will not begin until the student has received a minimum of 4 hours classroom instruction. BTW instruction must be completed no later than 3 weeks after the classroom instruction has been completed. The entire program must be completed in 6 weeks. Students who satisfactorily complete both phases of instruction will receive a Michigan SEGMENT 1 Driver Education Certificate. The SEGMENT 1 Driver Education Certificate must be taken to a Secretary of State office and a level one learner’s license must be obtained before a student can legally drive. Major Driving School LLC will provide the textbook and all the materials needed for the course work. The full cost of Segment 1 needs to be paid in full by the 1st day of class.

In the event of a BTW driving appointment cancellation, change, or no show, a cancellation fee or no show fee of up to a $50 may be assessed.

Notice Statement: This driver education provider is required to be certified by the Secretary of State. If you have any complaint that cannot be settled with the provider, please complete the Driver Education Complaint form found under “Driver Programs Division” on the Department of State website; www.michigan.gov/sos. Completing driver education does not guarantee a driver’s license.

RELEASE OF LIABILITY FORM

I, the undersigned, make this agreement as a participant in Segment 1, Segment 2, or a road test or as parent/legal guardian of undersigned. In consideration of me or the child’s participation in the driving instruction and/or road testing and any and all events or activities in relation thereto (collectively the “Activity”) permitted by Major Driving School, LLC and with the understanding that the me or the child’s participation in the Activity is only on the condition that I enter into this agreement for myself or the child, our heirs and assigns, I hereby assume the inherent and extraordinary risks involved in the Activity, and any risks inherent in any other activities connected with the Activity in which the me or my child may participate. I expressly assume the risk of and accept full responsibility for any and all injuries (including death) which may occur as a result of my or the child’s participation in the Activity and release from liability Major Driving School, LLC, its and their officers, directors, agents, representatives, heirs and employees. I hereby waive any and all claims I or the child may hereafter have as a result of any and all injuries disease or sickness (including death) to the me or the child as a result of the my or the child’s participation in the Activity. I hereby agree to indemnify all of the above named persons for any and all claims, including attorney’s fees and costs, which may be brought against any of them by anyone claiming to have been damaged as a result of any injury, sickness or disease (including death) to me or the child which may occur as a result of or during the Activity. I understand that the Activity may be dangerous and that physical injury, property damage or death may result. I certify that I have read and fully understand this release. I am of lawful age and legally competent to make this agreement. I understand that I am waiving any and all claims I or the Children may have against Major Driving School, LLC, its officers, directors, agents, representatives, heirs and employees as the result of participation in the Activity.
COVID19 RELEASE FORM

The coronavirus, COVID-19, SARS-Cov-2 is a highly contagious virus and there is a very high probability that you will be exposed to the virus while engaged in an activity with Major Driving School LLC. That being the case, we are asking that you carefully read and sign this release form.

I, the undersigned, make this agreement as road test or driver education applicant or parent/legal guardian of undersigned. In consideration of the road test applicant or driver education student, my/Children’s participation in the driving instruction and/or road testing and any and all events or activities in relation thereto (collectively the “Activity”) permitted by Major Driving School, LLC and with the understanding that my/Children’s participation in the Activity is only on the condition that I enter into this agreement for myself/the Children and myself, our heirs and assigns, I hereby assume the inherent and extraordinary risks involved in the Activity, and any risks inherent in any other activities connected with the Activity in which I/the Children may participate. I expressly assume the risk of and accept full responsibility for any and all injuries or decease (including death) which may occur as a result of my/the Children’s participation in the Activity and release from liability Major Driving School, LLC, its and their officers, directors, agents, representatives, heirs and employees. I hereby waive any and all claims I or the Children may hereafter have as a result of any and all injuries disease or sickness (including death) to myself/the Children as a result of my/the Children’s participation in the Activity. I hereby agree to indemnify all of the above named persons for any and all claims, including attorney’s fees and costs, which may be brought against any of them by anyone claiming to have been damaged as a result of any injury, sickness or disease (including death) to myself/the Children which may occur as a result of or during the Activity. I understand that the Activity may be dangerous and that physical injury, sickness from the property damage or death may result. I certify that I have read and fully understand this release. I understand that I am waiving any and all claims I or the Children may have against Major Driving School, LLC, its officers, directors, agents, representatives, heirs and employees as the result of participation in the Action.
Special Needs/Disabilities
Please inform the office manager and the instructor if you have any special needs such as a learning disability or a physical disability. An adult may read the 80 question test to a student if desired. Please let the instructor know. With your help, we will try to make accommodations that will help you succeed during your driver education class.
THE SECRETARY OF STATE ATTENDANCE POLICY

For both in person and online, the student is expected to attend all 12 class sessions. It you are late or don’t show for any part of a class, you will be counted absent and will need to make up that class.

It is a privilege to take the online Segment 1 class. This allows you to take driver education from the convenience of your home. That being said, due to the Secretary of State requirements for the online driver education classes, you will be required to adhere to the following Secretary of State Online Class guidelines at all times.

You will be expected to be present at every online class session. Your face must be shown on the screen the entire time to count as present. If the instructor is not able to see your face at any time, you will be counted absent for that video session and you will have to retake that day’s class. You should be sitting at a desk with little or no background noise or distractions. Laying in a bed or riding in a car are not permitted. For any absences, up to a $15 absence fee may be assessed.

Unless there is a family emergency, you will be expected to attend all 12 in person or online classes. It is extremely important that you and your parent/s or guardian/s are fully aware and committed to the attendance policy. If you are late to a class or miss a class, unfortunately, there may be a up to a $15 absence fee.

THERE IS A 6 WEEK DEADLINE
The Secretary of State requires that all coursework be completed within the 6 week period following the 1st day of class. Therefore, completion of the entire class is time sensitive. There is no option for extending the course work longer than this 6 week period. All road work, practice driving, makeup absences and the written testing must be completed in this 6-week period or the entire course will have to be taken over again at full price.
Road Work Reminders
1. Cancellations/Reschedules – 24 Hour Rescheduling Period. Each drive you receive is tentative. You will have 24 hours from when you receive a drive time to cancel or reschedule. If not changed within that 24 hour period, please keep the scheduled drive. Once the golden 24 hour period has passed, if you cancel or don’t show, there may be up to a $40 rescheduling fee.

2. On the day of your drive:
A. Please look for your instructor, listen for the car horn and be at the door waiting.
B. Be ready at least 15 minutes early.
C. Leave at least 30 minutes after the end of your drop off time. Your drive may go longer.
D. Stay at your pickup point unless your instructor tells you to leave. If at your high school, DO NOT get on the bus unless your instructor personally tells you to do so.
E. You will usually be scheduled in groups of 2 or more.
F. Your drive times are approximate times. Times may be adjusted slightly due to weather, traffic, or scheduling changes.
G. Your instructor will usually pick you up within 5 or 10 minutes of your pick up time but s/he may be up to 30 minutes late or on rare occasions, even 60 minutes late. Please do not assume your drive is canceled unless your instructor calls you and talks to you personally.
H. Your instructor may be late or early but will rarely cancel. Due to vehicle repairs, accidents, or unforeseen traffic delays, your instructor may have to cancel, but this will not happen often. We will be there for you.
3. Please bring your white “In Car Instructor Observations” form and your yellow “In Car Observation” form to ALL drives.
4. If possible, we will be able to pick you up at your house or we may ask you to meet us at another location, like the nearest McDonalds.
5. In some cases your road work will be done by the last day of written class work. Most of the time it will take an extra week or two after the written class is over. It will depend on the scheduling and class size. IF for some odd reason you don’t complete the classwork and practice driving in 4 weeks, it may take up to another 2 weeks, for a total of 6 weeks to complete.
6. If you miss a drive or need to reschedule, your make-up drives will need to be finished within 3 weeks after the last day of the S1 class.
Segment 1 Certificate Information
The student will soon be receiving a Segment 1 certificate of completion after s/he passes the Segment 1 class. Here are some important points to remember:
1. You MUST take the certificate of completion to the Secretary of State office to obtain your teen’s white LEVEL 1 LICENSE. Do this as soon as possible after receiving the certificate of completion.
2. Your child CANNOT legally drive with just the certificate of completion.
3. DO NOT delay in getting your teen’s white Level 1 license. Your teen needs to hold the Level 1 license for 6 months from the date of issue in order to take the road test or obtain a Level 2 license.
Security Camera Program
I give Major Driving School, LLC permission to use security cameras or cameras in the classroom and in the driver education automobiles. Some still shots may be posted on our website or social media if client and parent approve. All security camera files will be archived by Major Driving School, LLC. They will be kept private and will not be used for resale or public use.
You may be asked to give Major Driving School, LLC permission to post stills on their website or social media with parent approval.
ADDITIONAL FEES
Additional fees may be charged in the event of abusive property damage. This property includes the classroom location, classroom furniture and/or fixtures, automobile, and other materials which are part of the operations of Major Driving School LLC or any of the rented class rooms. Fees may be for repair or replacement as necessary. If the textbook is not returned in normal use condition, a $30 book replacement fee may be assessed. If at any time, a refund is requested, there may be up to a $20 service fee. Up to a $20 fee may be charged for any replacement certificate for S1, S2, or Road Test.
Driving Task Requires Maturity
Because the driving task requires maturity, proper behavior, appropriate attitudes, compliance with laws and cooperating with other drivers, these qualities must be developed or present in each student. Any student who displays an improper attitude towards authority, other students, or others involved in the classroom, may not be prepared to get behind the wheel of an automobile. In light of this, none of the following will be permitted in the classroom or near the vicinity of the classroom building: horseplay, inappropriate or disruptive behavior, an uncooperative spirit, a poor attitude, or misconduct of any kind. Any displays of such may result in expulsion. There will be no smoking at any time.
FAILURE OF WRITTEN WORK OR ROAD WORK
You must pass the 80 question written test and the road work in order to pass the course. You need to pass the written test with 70%, -24 or better. In the event you should fail the written test, you have the option to retake the written test to 2 times at no extra charge. If you should fail the written test after 2 retakes or the BTW, practice driving, you will need to take the entire Segment 1 course over again; (24 hours of class work and 6 hours of road work).
